Patient Attendance Procedure
The purpose of this document is to inform you of the procedure for your dental appointment (also hosted on our website). The patient attendance procedure is outlined below:
- For those patients who have smart phones and have consented will receive a SMS link the day before their appointment at Blagreaves Dental Practice. This link will allow access to the patient portal. The appointment may need to be cancelled if the 3 parts shown below are not completed before arriving:
- Go to ‘My Details’ highlighted in blue and check your personal details. Please ensure all your details are correct, particularly your mobile phone number and email address.
- Complete COVID Questionnaire marked in green.
- Complete the Medical History Questionnaire marked in green.
- The patient should use the toilet before attending the practice.
- The patient should bring their own pen.
- The patient should leave all belongings (including coats and bags) in the car. If the patient attends via another method then only minimal belongings should be bought.
- The patient should attend alone wherever possible or with one other person if absolutely necessary
- All patients should bring their own face covering when they attend
- If the patient requires a chaperone, they will be asked to leave the surgical room if an AGP procedure (fillings etc) is to be performed.
- On arrival, Patients will be asked to wait by the large sign directly in front of the car park entrance. The sign reads ‘please wait here’. This area also has cones to isolate it and help identify the area.
- The patient can only enter the building when summoned by their designated nurse who will come and collect them. If the patient is in the car park they will be visible to the team via CCTV. Please maintain 2M social distancing. You may also use the patient portal and check in. this will tell everyone at the practice that you are outside. To do this you will need to log back into the patient portal (SMS link would have already been sent). You will see a green box with 3 horizonal white lines at the top of the page- click this, go to ‘Appointments’ and click ‘Self Check-in’.
- The patient will be collected by a team member wearing appropriate PPE who will check their temperature and disinfect hands. A clear plastic bag will be given to the patient to place their mask into, while in the surgery. Please wait for the instruction of the clinician before you do this. The patient should interlace hand to avoid touching anything. The patient will be prompted by their nurse to do this.
- Patients will be screened using the standard COVID-19 screening questions.
- Any patient belongings will be placed in a sealed clear box in the secure reception area and will be disinfected after use. The nurse will take you to the designated surgery.
- At the end of treatment, the patient should disinfect their hands, place their mask on and follow their nurse to the designated reception area where they will be greeted by an experienced receptionist.
- The receptionist will ask the patient to disinfect their hands before and after completing any remaining paperwork, future appointments and payments (please avoid cash payments).
- The patient will collect their belongings and leave the practice via a designated exit point different to the entry, leading back to the car park.
